This digital service model is not clinically appropriate for patients who:

• Are under 18 years of age
• Are housebound
• Are safeguarded or considered vulnerable
• Are unable to use online based services
• Have a severe mental illness that requires regular or specialist in-person support
• Need regular face to face appointments with their GP surgery for any reason
